Index of SCAO Approved Forms for Use in
Appeals
This set of forms is for use in appealing matters from circuit, district,
and probate court. The forms must be filed in the appropriate court.
Note: Effective May 1, 2012, the following forms are temporarily not available for use because they do not comply with recent amendments to the Michigan Court Rules. Revised forms are being developed and will be available as soon as possible.
